Lessons learnt & safeguarding adults review sub group


The lessons learnt & safeguarding adults review sub group is responsible for ensuring that if things go wrong, organisations across the city can look at what lessons can be learnt to improve the practice of individuals of organisations.

City of York Safeguarding Adults Boards has developed Guidance for carrying out a general lessons learnt review & Safeguarding Adults Review in accordance with S44 of the Care Act 2014 (the legislation which covers adults’ safeguarding)

If the SAB has commissioned a Safeguarding Adults Review (SAR) it must provide information about these reviews and ensure it tells people what action it has taken in response to the findings of the review.

The Board are currently reviewing and updating its procedures for undertaking a lessons learnt review or safeguarding adults review to ensure its expectations for undertaking such reviews are clearly understood.  This will be added to the website during the month of March 2016.

The Board has not yet commissioned any Safeguarding Adults Reviews.
