Before you begin

Before you begin, please ensure you've read about how to raise a concern. When completing this online form you'll need to know certain information, some of which is 'required', meaning you can't continue the form without it. Gather the following details in the first instance:

• title, name and address (including postcode), telephone number and email address of person for whom you have concerns
• whether the person is aware you are raising this concern
• whether the person has any special communication needs, and the nature of those needs
• whether the person relies on help and support, and who provides it
• the details of your concern,
• your own contact details (name, telephone number and email address) and whether you're happy for these to be shared with third parties (or your reasons for wanting to remain anonymous)
